Gynecomastia in patients with chronic liver disease is caused by reduced catabolism of androstenedione. Many patients with liver cirrhosis take the diuretic spironolactone for cirrhotic ascites or for hypertension; spironolactone is a known inhibitor of testosterone synthesis and thus contributes to feminization in these patients.
